The only kind of adaptor I've ever considered is "wobble bolts" for FIAT/Alfa cars which have a really silly 4x98mm PCD. Never used them mind.

Spacers and PCD adaptors of this kind from reputable companies, can be very expensive ... and because of this there's a strong market for cheaper ones from eBay from China, so I'm tempted to speculate that some of these failures are down to combination of 1. poor quality parts 2. incorrectly fitted by people with limited common sense or limited mechanical ability. The spacer itself is a lump of metal so there's not really a lot that can go wrong, but if supplied with cheap bolts or incorrect length then I can see there being an issue.
Like anything the more joints you have the more failure points you introduce, so personally it's not something I'd want to run with I've been in that there VW scene for a number of years and haven't seen or heard of anyone having issues. Perhaps the people I know know how to torque a bolt?
